By submitting this request and checking the box below, I authorize Metrum Community Credit Union to pay, on my behalf, each balance or portion of balance I have designated. I have read and agree to the terms and conditions noted below.

TERMS AND CONDITIONS
PROMOTIONAL BALANCE TRANSFER OFFER The Annual Percentage Rate (APR) for Balance Transfers will be 5.99% for 12 months. After the expiration of the introductory rate period, Your APR will vary with the market and will be determined in accordance with the terms of Your original Agreement and Disclosure. Balance transfer transactions will accrue finance charges at an introductory Daily Periodic Rate of 0.01641% (corresponding to an ANNUAL PERCENTAGE RATE of 5.99%) for 12 months. Upon expiration of the introductory rate period, any remaining balance transfer balance will accrue interest at the applicable non-promotional APR disclosed in Your Credit Card Agreement and Disclosure. A Balance Transfer Fee of 3.00% of each transfer will apply, with a minimum fee of $5.00 per transfer. TERMS AND CONDITIONS Balance transfers are subject to the terms and conditions of Your Credit Card Agreement and any applicable promotional offer. The amount of the transfer(s) cannot exceed Your available credit limit. Please allow up to three (3) weeks for processing and continue to make payments on Your existing credit card accounts until the balance transfer is confirmed. Submission of this form does not constitute payment to the accounts listed above. You remain responsible for making all required payments to Your creditors while the requested balance transfer is being processed. Metrum Community Credit Union is not responsible for any remaining balance on the account or for any finance charges, fees, penalties, or other charges incurred as a result of delays in processing the balance transfer. If You wish to close an account being paid by a balance transfer, You must contact the creditor directly. Balance transfers may not be applied to existing Metrum Community Credit Union loan or credit card balances.
