Parent/Guardian Consent for Students

Age 17 & Under

If you are age 17 or under, we will need the permission of your parent/ guardian for you to apply for the scholarship program. Please download and complete the Page 2 of the scholarship package. Then, upload the completed form. You can upload the form as an image (JPG, PNG) or document (PDF).

Name/Photo/Essay Release

Scholarship recipients are announced on COPFCU social media channels, the COPFCU website, through email, etc. As a result, please complete the Name/Photo/Essay release form.

Proof of Enrollment

Upload proof of acceptance / enrollment for the 2025-2026 term. You may upload an image (JPG or PNG) or document (PDF).

Proof of Cumulative GPA of 2.5 or Higher

Upload proof of your cumulative GPA of 2.5 or higher for the most recent school term (whether high school, college or vocational/trade school). You may upload an image (JPG, PNG) or document (PDF). Proof submitted must include your name, and the most recent school term.

Credit unions often play a vital role in promoting financial well-being within communities. Describe your understanding of financial well-being and the unique benefits that credit unions offer compared to traditional banks. How has your experience with or knowledge of credit unions shaped your perspective on financial health? Discuss how you plan to leverage this understanding to enhance your own financial future and contribute to the financial wellness of others in your community.
