REJECTION OF TERM CHANGES - DMCU Visa® Credit Card

By completing and submitting this form, you are electing to reject the recent changes to your DMCU Visa® Credit Card terms, which include:

• A Returned Payment Fee of up to $30 per returned payment

• A Balance Transfer Fee equal to 2% of the amount of each balance transfer

Important Notice: If you choose to reject these changes, your DMCU Visa® Credit Card will be closed and you will no longer be able to use it for new transactions. However, you will remain legally obligated to repay any outstanding balance on your account under the terms of your existing cardholder agreement until the balance is paid in full.
