I, the undersigned, being over the age of 18, give Chiphone Federal Credit Union and its agents and assigns, (collectively “Credit Union”) the right to use information concerning me, including my first name, first letter of last name, the fact that I am a Credit Union member, my profile, and any statements made by me (collectively the “Profile”) to use, broadcast, or publish on any of Chiphone Federal Credit Unions’ web sites, social media, YouTube or in any other print, broadcast or electronic medium and for any purpose whatsoever (the "Permitted Uses").

I understand I will not receive any monetary compensation and I waive any right that I may have to inspect or approve the finished product that may be used in connection therewith or the use to which it may be applied.

I hereby release and discharge the Chiphone Federal Credit Union, its agents, employees, officers, successors or assigns from any and all claims and demands I may now or hereafter have arising out of or in connection with the Permitted Uses of the Profile, including, without limitation, any and all claims for the right of privacy, publicity, libel or slander and any and all claims and/or demands for royalties associated therewith.
