Opt-In to Authorize BlueOx CU to pay overdrafts on ATM and everyday Debit Card Transactions

I/We authorize BlueOx Credit Union to pull my/our credit report for the purposes of qualifying me/us for a BlueOx Credit Union Checking Account. I/We also certify, by checking the box to the left that I/we have read and understand the disclosures above. I/We understand that I/we will receive a free MasterCard® Debit Card in each applicant’s name when I/we open a BlueOx Credit Union Checking Account. A Member Service Advisor may contact you in relation to this application for additional information and other potential savings we may have available for you.

I/we agree to the terms and acknowledge receipt of the Member Service Agreement, Truth-In-Savings Disclosure and Overdraft Protection Disclosure, and to any amendment the Credit Union makes from time to time which are incorporated herein. Under penalties of perjury, I certify that: (1) The number shown on this form is my correct taxpayer identification number; (2) I am not subject to backup withholding because: (a) I am exempt from backup withholding, or (b) I have not been notified by the Internal Revenue Service (IRS) that I am subject to backup withholding as a result of a failure to report all interest or dividends, or (c) the IRS has notified me that I am no longer subject to backup withholding; and (3) I am a U.S. person (including a U.S. resident alien).
