DISCLOSURE AND AGREEMENT

I hereby request Media City Credit Union increase my ATM/Debit Visa limit(s) to the amount(s) requested. I agree that this ATM/Debit Visa card limit increase request is subject to the approval of Media City Credit Union and is therefore not guaranteed by the submission of this form. I authorize Media City Credit Union to obtain a credit bureau report, if necessary. This change may be subject to periodic review and the limits may be changed at the discretion of Media City Credit Union without previous notice.

HOLD HARMLESS & RELEASE OF LIABILITY

I agree to indemnify, defend, and hold the Media City Credit Union and its employees harmless from and against every claim, demand, action, cost, loss, liability, and expenses including, without limitation, attorney’s fees incurred by acting in accordance with Media City Credit Union’s Account Agreement and Truth-In-Savings Disclosures or as a result of my failure to abide by its terms.
