For Organizational Accounts with more than two debit card holders:

Enter Card Holder's Name, Last 4 digits of Debit Card, and Design Choice. Separate each card holder by semi-colon.

Please be on the lookout for your new debit card. It will arrive in a plain white envelope. You can activate the card immediately, however, it will not be connected to your checking account until May 14th. Use your current debit card through May 13th.
